Enrollment Period:

- Ends: Two full months after the month of notification.
This SEP gives you a one-time opportunity to change plans, whether switching to another Medicare Advantage Plan (with or without Part D) or returning to Original Medicare with the option to enroll in a stand-alone Part D plan.

- Coverage begins the first day of the month following the submission of your completed enrollment application.
